Vestry Member Form of Declaration

The form of declaration is available to download here.

A person who wishes to join the General Vestry of a parish or district. will need to confirm that he or she is a member of the Church of Ireland aged 18 or over, is usually resident at a specified address, and whether or not he or she resides in the parish.
