If you have more than one Gift Aid contact, please complete separate forms for each individual and return them to the RCB either by email or by post to Gift Aid, Church of Ireland House, Church Avenue, Rathmines, D06 CF67, Ireland.
You can request hard copies of declaration forms by contacting the accounts department. We will be happy to send you as many forms as you require by post.
Alternatively, forms may be downloaded in PDF format below. Ideally, these forms should be printed double–sided so that the notes on the back are accessible to the individual.
